PRIVACY CONSENT: All players entering The Greatflood AAA Hockey Classic 2008 Tournament will have their name and jersey number published in the Tournament Program to be sold at the Tournament. If you do not want a childs name and number to be published in the Program, please contact the Tournament Coordinator, Alan Aime at:(204) 738-2723 or by e-mail: aime.farm@qstream.com by no later than May 7, 2008. If you do not contact the Tournament Coordinator with instructions not to publish this information, your team officials and parents will be deemed to have consented to such publication.

WAIVER: In consideration of being allowed to participate in the 2008 Great Flood AAA Hockey Classic, related events, and activities, the undersigned acknowledges and agrees, on behalf of the attending team that The Great Flood AAA Hockey Classic will not be held responsible for any accident, injury, or loss however caused and hereby releases and holds harmless the Great Flood AAA Hockey Classic, their officers, directors, officials, coaches, agents and/or employees, other participants, arena officials and owners, sponsors, and advertisers, with respect to any and all injury, disability or loss to person or property, whether caused by the negligence of the releases or otherwise.
